The following events have forms which will be posted and available on this website. Forms for any event which is hot linked can be downloaded and printed. Forms which are not hot linked are not currently available. Contact the Windy City office for more information. 2008 Medical Release Form - This form is mandatory for all players in any Windy City event. All players must fill in all information and bring it to the first day of the event in order to participate. Forms for Open Registration Events: Anyone who qualifies by age may download, print and sent in to the Windy City office for as they become available.
Forms for Select Team Registration Events: These forms are only for players that have been selected to the Windy City Select Teams.
