The following events have forms which will be posted and available on this website. Forms for any event which is hot linked can be downloaded and printed. Forms which are not hot linked are not available from the web site and the Windy City office should be contacted for more information. 2009 Medical Release Form - This form is mandatory for all players in any Windy City event. All players must fill in all information and bring it to the first day of the event in order to participate. You must contact the Windy City office to receive this form. Forms for Open Registration Events: Anyone who qualifies by age may download, print and sent in to the Windy City office for as they become available.
Forms for Select Team Registration Events: These forms are only for players that have been selected to the Windy City Select Teams. They are not posted on the web site. All forms are mailed or e-mail to the Select Team players.
